The DSM-5 Substance Use Symptom Checklist is completed by patients on paper or online and used by the primary care provider to engage patients in discussing their symptoms. It has been psychometrically validated in English. For use in making a diagnosis, clinicians must confirm that symptoms are recurrent in their patient interview. See publications.
